QURE4U PLATFORM AND SERVICES PRIVACY POLICY 

All capitalized terms herein shall have the same meaning as defined in the Qure4u Platform and Services Patient Terms of Use. This Privacy Policy (this “Policy”) discloses the privacy practices for the Qure4u Platform and Services. Qure4u is committed to protecting your privacy online. Please read the information below carefully to learn our practices for collecting, using, maintaining, protecting, and disclosing information regarding your use of the Platform and Services.

You acknowledge that this Policy is in addition to the Terms of Use, and by accessing or using the Platform and Services, you agree to be bound by the Terms of Use and this Policy (collectively, the “Terms”). If you do not agree to the Terms, you may not access or use the Platform and/or Services. Any capitalized term not defined herein shall have the meaning ascribed to it under the Terms of Use.

Qure4u reserves the right to change this Policy at any time. Such changes, modifications, additions, or deletions shall be effective immediately upon Qure4u’s posting of the revised Policy on this page. Your continued use of the Platform and Services after such modifications will constitute your: (a) acknowledgment of the modified Policy; and (b) agreement to abide and be bound by the modified Policy.

1. What Types of Information do we Collect?

In connection with the provision of the Platform and Services, Qure4u may collect certain types of information about its users and their use of the Platform and Services, which may include personal data. Qure4u’s primary goal in collecting information from you is to provide you with a smooth, efficient, and customized experience while using the Platform and Services. 

What is Personal Data?

Personal data refers to information that lets us know the specifics of who you are. When you engage in certain activities on the Platform and Services, Qure4u may ask you to provide certain information about yourself by filling out and submitting an online form. Such activities may include:

• Registering for a membership;
• Ordering a product or service;
• Submitting content, for example, demographic information, but not limited to, and electronic patient health Information;
• Posting content in discussion forums, texting, or other public areas;
• Entering a contest or sweepstakes;
• Filling out a survey; or
• Sending us feedback.

It is completely optional for you to elect to engage in these activities, however, if you do, Qure4u may ask that you provide us personal data, such as your:

• First and last name;
• Mailing address (including zip code);
• E-mail address;
• Employer, job title, and department;
• Telephone and facsimile numbers;
• Credit card information when used in connection with the Services; and
• Other personal identifying information as may be required in order for us to provide Services.

2. How do we Collect and Use Information? 

Qure4u does not collect any personal data about you unless you or a Registered Provider provides it to us. In addition, Qure4u may collect, and/or our third-party ad server and/or content server may collect, certain data that is ultimately stored in the form of store categories, and, in some cases, specific URLs.

Qure4u uses your IP address to diagnose problems with our servers, software, to administer the Platform and Services to you, and to gather demographic information. Our third-party ad servers will also provide us with reports that tell us how many ads were presented and clicked upon on the Platform and Services. Qure4u will primarily use your personal data to provide our services to you via the Platform and Services or otherwise, as required by our agreements with your Registered Provider. Qure4u will also use personal data to enhance the operation of the Platform and Services, including the use of de-identified Health Data, fill orders, improve our marketing and promotional efforts, statistically analyze the Platform and Services use, improve our product and service offerings, and customize the Platform and Services’ content, layout, and services. Qure4u may also provide non-personal data about our customers’ sales, traffic patterns, and related Platform and Services information to third party advertisers, but these statistics do not include any personal data.

Depending upon your use case(s), some of the information Qure4u asks you to provide may be identified as mandatory, while other information may be identified as voluntary. If you do not provide the mandatory data with respect to a particular activity, you will not be able to engage in that activity. 

3. How and When do we Release Information? 

Qure4u does not sell, trade, or rent your personal data to others. Qure4u does provide some of our services through contractual arrangements with affiliates, services providers, partners and other third parties. Qure4u and our service partners only use your personal data to operate and deliver the Platform and Services. For example, Qure4u may be required to:

• Release your credit card information to the card-issuing bank to confirm payments related to the Platform and Services;
• Release your address information to the delivery service to deliver products that you ordered; and
• Provide order information to third parties that help us provide customer service.

Qure4u has adopted measures designed to share no more personal data than is necessary to provide the Platform and Services is disclosed to third parties. In addition, Qure4u actively encourages our service partners to adopt and post their own privacy policies, however, the use of your personal data by our service partners is governed by the privacy policies of those service partners and is not subject to our control.

Qure4u may disclose your personal data as reasonably necessary to comply with any court order, law, or legal process, such as, to respond to any governmental or regulatory request. If Qure4u receives such a request, it will take reasonable steps to notify you, provided that the applicable law or governmental or regulatory request allows notice. Qure4u fully cooperates with law enforcement agencies in identifying those who use our services for illegal activities. Qure4u reserves the right to report to law enforcement agencies any activities that Qure4u in good faith believe to be unlawful.

Qure4u may also provide non-personal data about our customers’ sales, traffic patterns, and related Platform and Services information to third party advertisers, but these statistics do not include any personal data. 

4. How Can You Update and Correct Your Information?

Qure4u believes you should have the ability to access and edit the personal data that you have provided to us. You may change any of your personal data at any time by linking to your account in accordance with instructions posted elsewhere on the Platform and Services. You may also access and correct your personal data and privacy preferences by writing us at:

Qure4u encourages you to promptly update your personal data if it changes. You may ask to have the information on your account deleted or removed; however, because Qure4u keeps track of past transactions, you cannot delete information associated with past transactions on the Platform and Services. In addition, it may be impossible to completely delete your information without some residual information because of backups. 

5. User Choices on Collection and Use of Information 

Qure4u may, from time to time, send you e-mail or text messages regarding our products and services. In addition, if you indicated upon registration that you are interested in receiving offers or information from us and our partners, Qure4u may occasionally send you direct mail about products and services that Qure4u feels may be of interest to you. Only Qure4u (or agents working on behalf of Qure4u and under confidentiality agreements) will send you these direct mailings and only if you indicated that you do not object to these offers. If you do not want to receive such mailings, simply tell us when you give us your personal data. At any time, you can easily edit your account information to no longer receive such offers and mailings.

6. How do we Secure your Personal Data? 

When using the Platform and Services, you can be assured that we implement administrative, technical, and physical safeguards that are designed to ensure that your personal data is secure. The importance of security for all personal data associated with our users is of the utmost concern to us. Your personal data is protected in several ways, including:

• Limiting unauthorized access to your personal data through a unique password and customer ID selected by you;
• Password encryption;
• Ensuring that your personal data you provide resides on a secure server that only select Qure4u personnel and contractors have access to via password; and
• Encrypting your personal data.

In order to most efficiently serve you, credit card transactions and order fulfillment are handled by established third party banking, processing agents, and distribution institutions. They receive the information needed to verify and authorize your credit card or other payment information and to process and ship your order.

Unfortunately, no data transmission over the internet or any wireless network can be guaranteed to be 100% secure. While Qure4u has implemented administrative, technical, and physical safeguards that are designed to protect your personal data, you acknowledge that: (a) any transmission of personal data is at your own risk; (b) there are security and privacy limitations of the internet which are beyond our control; (c) the security, integrity and privacy of any and all information and data exchanged between you and Qure4u through the Platform and Services cannot be guaranteed; and (d) it is impossible to eliminate all risk that any such information and data may be viewed or tampered with in transit by a third party. 

7. Third Party Advertisements

Qure4u may use an outside ad service company to display banner advertisements on the Platform and Services. Qure4u will never provide any third-party ad server with any of your personal data or information about your purchases without your consent. Qure4u and our third-party ad server may collect and use non-personal data about you, such as:

• The server your device is logged onto;
• The area code and zip code associated with your device; and
• Whether you responded to a particular ad.

Other advertisers may also place banner ads on the Platform and Services in the same manner as above, but Qure4u will not disclose any personal data to them. 

8. Privacy Policies of Third-Party Sites 

Except as otherwise discussed in this Policy, this document only addresses the use and disclosure of information Qure4u collects from you. Other websites accessible through the Platform and Services have their own privacy policies and data collection, use, and disclosure practices. Please consult each site’s privacy policy. Qure4u is not responsible for the policies or practices of third parties. Additionally, other companies which place advertising on the Platform and Services may collect information about you when you view or click on their advertising through the use of cookies. Qure4u cannot control this collection of information. You should contact these advertisers directly if you have any questions about their use of the information that they collect. 

9. Passwords

You must choose a secure password when using the Platform and Services and you should not divulge your password to anyone. You are solely responsible for any activity on your account and for maintaining the confidentiality and security of your password. Qure4u is not liable for any acts or omissions by you or any third party who accesses the Platform and Services through your account. 

10. Miscellaneous Privacy Issues 

It is our policy to not knowingly collect, use, or disclose personal data about the Platform or Services users who are under 18 years of age.

You should also be aware that when personal data is voluntarily disclosed (i.e., your name, E-mail address, etc.) in the discussion forums or other public areas on the Platform and Services, that information, along with any information disclosed in your communication, can be collected and used by third parties and may result in unsolicited messages from third parties. Such activities are beyond our control and this Policy does not apply to any such information that you may disclose. Any submissions to chat rooms or other public areas on the Platform and Services are accepted with the understanding that they are accessible by third parties. If you do not want your comments in discussion forums to be viewed by third parties, you are advised not to make any forum submissions.
